Your New Patient Guide
Has your dentist recommended that you visit a periodontist? Are you tired of dealing with gum disease or tooth loss and want an expert to take care of it right away? It can be a bit nerve-racking to set up an appointment if you haven’t visited a dental specialist before, but this page will cover everything you need to know beforehand. Just read it over, and if you have any questions, you are always welcome to reach out to our team. We want you to feel at ease from the moment you step through our door!
Your First Visit
For everyone’s safety, you will be screened for Covid-19 when you enter our practice, and we do require everyone to wear a facemask. We’ll start by going over your medical history (which you’ll share by filling out our forms), and then we’ll perform an exam and take some X-rays. From there, your periodontist will discuss their findings, propose a treatment plan, and answer any questions you might have. Once you have both agreed on the next steps, you’ll be brought to our treatment coordinator to work out the details.

For your convenience, we have posted all New Patient forms online. You can now download, print, and complete these forms at home. Please bring all of them with you to your first appointment. Should you have any trouble downloading these forms, please call our office, and a staff member will be happy to assist you.
Dental Insurance
At our office, we make every effort to provide you with the finest surgical care and the most convenient financial options. To accomplish this goal, we’ll work hand in hand with you to maximize your insurance reimbursement for covered procedures. If you have any problems or questions, please ask our staff.

They are used by thousands of healthcare providers around the country, and signing up only takes a few minutes. Payment Options
For your convenience, we accept Visa and MasterCard. We deliver the finest care at the most reasonable cost to our patients, therefore payment is due at the time service is rendered unless other arrangements have been made in advance. If you have questions regarding your account, please contact us at our office during normal business hours. Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage. We will send you a monthly statement. Most insurance companies will respond within four to six weeks. Please call our office if your statement does not reflect your insurance payment within that time frame. Any remaining balance after your insurance has paid is your responsibility.
